The hormonal process of testosterone converting to DHT, which then harms hair follicles, happens in both men and women. Estrogen, when lowered as commonly seen in menopause, creates a change in the ratio of male to female hormones, giving an edge to these male hormones. Compounded with the sensitivity of DHT to the hair follicles, heredity can affect the age at which a woman begins to lose her hair, as well as the rate of hair loss and the extent of baldness. 
Optimal levels of estrogen help to grow full thick hair, while low estrogen levels lead to thin and stalled hair growth, which eventually leads to hair loss. Through the years, a women will go through various cycles of highs and lows in estrogen levels. Puberty is typically associated with high estrogen levels as a woman starts to cycle through menstruation. Pregnancy typically increases hormone production, but once the baby has been delivered the mother can experience a drop in estrogen levels, which can result in thinning and loss of hair. But other common nutrient deficiencies can arise as part of a less than stellar diet low in zinc, protein (especially the amino acid lysine) or a deficiency of vitamin B6 (a very common issue with women taking hormones like the birth control pill). Low vitamin D has also been linked to excessive hair loss as has excessive vitamin A intake (less common.)

This article was very informative for me. I am a 56 yr. old male struggling with high estrogen levels and low T. My last blood test taken last week my estrogen level was 100 and my T was 700 its climbing because I am taking testosterone inj. Twice a week at 50mg per inj. I’m also taking Anastrozole 1/10 tab 5 days a week. I just started on that. Its only been 2 days for the estrogen blocker. My belly fat is increasing and I’m feeling and looking extremely bloated. I’m depressed because of the weight gain and the way I now look. My healthy weight is 153 I’m 5’8″ but now I’m 178lbs. I’m also constipated and feel there is a lot of food not passing through, please let me know which part of this is normal and will the Anastrozole start working gradually and cut my belly fat out? My diet is high fiber, low carbs. I am working on working out more often. I’m a non smoker and non drinker.

A study of women after 3 years of therapy showed that dutasteride may be more effective than finasteride in women under 50 years of age as measured by hair thickness (not hair density) at the center and vertex scalp (Boersma et al., 2014). One case report of a 46-year-old female with FPHL showed some response after 6 months of treatment with a dose of 0.5-mg dutasteride daily despite a minimal response to treatment with finasteride and minoxidil (Olszewska and Rudnicka, 2005). Data with regard to the treatment side effects in women is extremely limited. Dutasteride is classified as pregnancy category X because of teratogenicity and should have the same theoretical risk of breast cancer as mentioned in relation to finasteride (Kelly et al., 2016).

Both of these hairy issues are related to excess androgens that are part of PCOS.  Our hair follicles respond overzealously to testosterone creating both hair loss and hair growth. This is because the follicles on the face tend to stretch and grow causing the hair to get stuck and not fall out when they should. Conversely, the scalp hair follicles actually shrink in response to androgens and the hair can fall out or just not be as robust.
The normal cycle of hair growth lasts for 2 to 6 years. Each hair grows approximately 1 centimeter (less than half an inch) per month during this phase. About 90 percent of the hair on your scalp is growing at any one time. About 10 percent of the hair on your scalp, at any one time, is in a resting phase. After 2 to 3 months, the resting hair falls out and new hair starts to grow in its place.

It’s natural for estrogen levels to fluctuate throughout a woman’s life, but drastic drops can disrupt the hair growth cycle and cause telogen effluvium. For example, during pregnancy, a woman’s estrogen levels are at their peak. Fewer hair follicles enter the telogen stage of the hair cycle, so hair looks thicker than usual. A few months after pregnancy, however, estrogen levels drop and the excess hair falls out. While totally normal, this type of telogen effluvium can be very upsetting for new mothers.
Reproductive hormone changes can also play a part, and dermatologists believe the same factors that cause hair loss from the scalp, a common problem for women as they age, may contribute to thinning brows. Nanette Santoro, M.D., ob/gyn and professor of reproductive endocrinology at the University of Colorado at Denver, says, “Abrupt hormone changes can cause sudden hair loss (telogen effluvium) that recovers over about six months' time. It happens postpartum to many women and can happen at menopause.”

A new entity variously called as trichodysplasia spinulosa,[128] trichodysplasia of immunosuppression,[129] and cyclosporine-induced folliculodystrophy[130] has been described in immunocompromised patients, usually organ transplant recipients on immunosuppression. It involves the development of alopecia predominantly of the face with indurated spinous papules. There is a profound loss of eyebrows[131] and sometimes eyelashes. The histopathologic picture is that of abnormal follicles with hyperkeratotic infundibula and absence of normal hair shafts. The inner root sheath epithelium showed proliferation in the cells and dystrophic trichohyaline granules. Electron microscopy of skin showed presence of intracellular viral particles.[132] This entity has lately been reported in immunosuppressive states in patients without organ transplantation such as leukemias and lymphoma.[133–136] van der Meijden et al. described the discovery of a new polyoma virus in a patient with trichodysplasia spinulosa.[131] Histopathological examination can reveal the diagnosis. A recent simple pull-test wherein the spicules can be plucked and examined under the microscope for inner root sheath keratinization has been described.[136] Some successful treatments described are cessation of cyclosporine therapy[130] and oral valganciclovir[137,138] and topical cidofovir.[131]

It can be caused by any emotional or physiological stress, for example acute or chronic illnesses, hormonal changes, nutritional deficiencies, and medications that alter the normal hair cycle and cause the hair the enter the telogen phase prematurely.
Cyproterone acetate works in several ways. It not only competitively blocks DHT from binding to its receptors at target tissue (Gilman et al., 1990), but it is also a progestogen that lowers testosterone levels by decreasing the release of luteinizing and follicle-stimulating hormones through pituitary-mediated supression (Gilman et al., 1990, Varothai and Bergfeld, 2014). An open intervention study of 80 women who received treatment with spironolactone (200 mg daily) or cyproterone acetate (50 mg daily or 100 mg for 10 days per month if premenopausal) showed that three of four patients demonstrated an improvement or stabilization of their disease with no difference of effect between the therapies received (Sinclair et al., 2005).

Eyebrow thinning that manifests in irregular patches, especially accompanied by red and itchy skin, may indicate atopic dermatitis. This condition—commonly called eczema—can occur just about anywhere on the body and is usually hereditary. People with atopic dermatitis may be more sensitive to topical irritants, but eczema just from topical irritants is called allergic contact dermatitis or irritant contact dermatitis. While you wait on the appointment with your dermatologist, you can help ease eczema around the eyebrows by switching to mild soaps or a soap-free facial cleanser, and applying an intensive facial moisturizer within three minutes after bathing.
Topic prostaglandin analogues are used for the treatment of glaucoma. Uniocular increase in length, thickness, and pigmentation of the lashes were described by Johnstone in 1997 by patients using latanoprost in one eye.[184] There are reports of a response of alopecia of eyelashes to cutaneously administered latanoprost,[185,186] though others report limited success.[187] Eyelash growth has also been reported following bimatoprost 0.03% topically to the base of the eyelashes in healthy individuals[188–190] and in individuals with alopecia areata.[191] Bimatoprost ophthalmic solution 0.03% is the only Food and drug administration (FDA)-approved product to safely and effectively enhance the growth of a patient's own eyelashes.[192,193]
Reproductive hormone changes can also play a part, and dermatologists believe the same factors that cause hair loss from the scalp, a common problem for women as they age, may contribute to thinning brows. Nanette Santoro, M.D., ob/gyn and professor of reproductive endocrinology at the University of Colorado at Denver, says, “Abrupt hormone changes can cause sudden hair loss (telogen effluvium) that recovers over about six months' time. It happens postpartum to many women and can happen at menopause.”

Like with low thyroid, there can be a myriad of imbalances at play with PCOS related hair loss making it a complex issue that’s often written off as merely high testosterone. While androgen excess and hypersensitivity of the hair follicles to these hormones are part of the issue, these are other contributing factors to PCOS related hair loss as well:
It takes more than just an apple a day to keep the doctor away; if your diet lacks the key vitamins A, B, D and E or nutrients such as iron, calcium or the amino acid L-lysine, your hair may suffer. Nutrient deficiency doesn’t just affect the eyebrows—it may lead to hair loss on the scalp, feelings of fatigue and physical weakness, lightheadedness or inhibited concentration, heart palpitations and pale skin, among other serious symptoms.
Hair loss is something that everyone experiences sooner or later, but some get it earlier than others. Losing hair prematurely can be traumatic. Because of that, there is an infinite number of products that claim to cure and reverse the problem. But what if you could find out the likelihood that you will experience hair loss so that you could prevent it before the symptoms even showed? Researchers in the United Kingdom have reportedly found a way to predict who is at risk.

Reduce stress. When you are stressed, your body starts shutting down processes that aren’t necessary for survival. This shutdown can occur with physical stressors, like surgery or an illness, and emotional stress, which often manifests as physical symptoms. Loss may occur up to three months after the stressful event and may take another three to grow again.[24]
For now, therapies include cortisone injections directly into the bald patches; topical cortisone; Minoxidil — known to many people under the brand name Rogaine; and anthralin cream. A less-widely available option is topical immunotherapy: certain chemicals applied to the scalp can trigger an allergic rash, which alters the immune response, NAAF notes.
The complex actions of genetics, DHT, shifting of hormone ratios and age-related volume loss can commonly occur in women in their 40’s and 50’s. However, just like in men, genetic hair loss can appear at all ages after puberty.  In fact, hair loss occurs with relatively high frequency even in women in their 20’s and 30’s. The majority of women with female pattern hair loss initially develop diffuse thinning over the front and top of the scalp, while maintaining the frontal hairline. This thinning may present with a widening through the central part line while others may present initially with either episodic or continuous hair shedding, prior to any noticeable decrease in hair volume. In addition, thinning may also be seen throughout the scalp, including the temple areas as well as the back and sides.

There are a lot of myths out there about balding men. One of them is that men with MPB are more virile and have higher levels of testosterone. This isn’t necessarily the case. Men with MPB may actually have lower circulating levels of testosterone but higher levels of the enzyme that converts testosterone to DHT. Alternately, you may simply have genes that give you hair follicles that are highly sensitive to testosterone or DHT.
Most women with pattern hair loss don't get a receding hairline or bald spot on top of the scalp as is common in men. Instead, there is visible thinning over the crown. In men and women, hairs are miniaturized because of a shortened growth cycle where the hair stays on the head for a shorter period of time. These wispy hairs, which resemble forearm hairs, do not achieve their usual length.
Scurf refers to the scales and greasy crusts that accumulate along the hair shaft and indicates the presence of seborrhea.[26,27] Collarettes are composed of hard fibrinous scales[25,28] surrounding each individual eyelash. They travel upward along with the growth of the lashes and are indicative of staphylococcal infection. Sleeves or cylindrical dandruff comprise scales that form a cuff around the lash root and are connected with it, in contrast to greasy scales which are not connected to the lash root.[29] Sleeves indicate infestation with Demodex folliculorum.

There is some evidence of a link between baldness and prostate cancer and other diseases. Harvard Medical School reports that men with vertex baldness have 1.5 times more of a risk of developing prostate cancer than men without bald spots. The risk of coronary artery disease is also more than 23 percent higher in men with vertex bald spots. Investigations are ongoing as to whether there is a link between DHT levels and metabolic syndrome, diabetes, and other health conditions.

Here is one small European study which looked at 20 pre-menopausal women with female pattern hair loss to check their serum levels of estradiol, free and total testosterone, SHBG, LH, FSH, and DHEAS. These levels were compared to those of a control group without hair loss. Estradiol is one of the three naturally occurring forms of estrogen found in the body.

But, interestingly — and what most people don't initially realize — is that eyebrow hair loss is often a sign of an internal disease. The classic example of this is an under-active or over-active thyroid. Loss of the outer third of the eyebrow, in particular, is a characteristic sign of an under-active thyroid. Fortunately, thyroid conditions are easily treatable, usually with oral medications, but the medications generally have to be taken for life. The eyebrows should grow back, but there is always the possibility that they may not. The thyroid gland plays a huge role in our bodies, so it is important to have any thyroid issues treated.

During pregnancy, high concentration of estrogen levels in women result in the development of thicker, stronger, longer and healthier looking hair. Following pregnancy however, women tend to fret and think that they’re losing their hair when in fact they’re merely shedding the excess hair that the excess oestrogen created. The hair that was in a prolonged anagen stage are now all shifting into telogen (resting phase) to make way for new hair growth. This type of hair loss is comes under the condition known as telogen effluvium but the good news is that when oestrogen levels have returned to normal, your hair will too.
Contact dermatitis causes skin inflammation and when severe it can affect the hair follicles as well. It can either be due to some irritant making contact with the skin of the eyebrows or due to a substance to which a person has an allergy. This gives rise to the two types of contact dermatitis – irritant contact dermatitis or allergic contact dermatitis. Substances that could cause contact dermatitis at the eyebrow includes eye makeup including eyebrow pencils, hair removal creams and waxes, other cosmetics, hair dyes and scalp hair styling products.
Optimal levels of estrogen help to grow full thick hair, while low estrogen levels lead to thin and stalled hair growth, which eventually leads to hair loss. Through the years, a women will go through various cycles of highs and lows in estrogen levels. Puberty is typically associated with high estrogen levels as a woman starts to cycle through menstruation. Pregnancy typically increases hormone production, but once the baby has been delivered the mother can experience a drop in estrogen levels, which can result in thinning and loss of hair. Typically, this corrects itself as the woman's body heals and hormonal regulation returns to normal.  The largest decline in estrogen levels is during menopause. Estrogen is secreted through the ovaries and adrenal glands. When a woman hits menopause, the ovaries’ estrogen production is significantly diminished, which puts an excess burden on the adrenals to produce it. However, as the adrenals are typically overburdened due to stress, symptoms of menopauses like hair loss and hot flashes occur prematurely and excessively for some. Nourishing the body and balancing stress levels will support gentle and graceful transitions.  

Madarosis is the hallmark of lepromatous leprosy. It was reported in 76% of patients with multibacillary leprosy.[76] Bilateral symmetric cicatricial madarosis occurs in lepromatous leprosy due to histiocytic infiltration of hair follicles[77,78] [Figure 4]. It occurs in multibacillary leprosy after at least 5 to 10 years of untreated disease.[79] Loss or atrophy of the eyelashes may follow. Madarosis adds to the cosmetic disfigurement caused by leprosy. Absence of madarosis is a good prognostic sign in long-standing cases.[80] Unilateral madarosis may occur in tuberculoid leprosy due to the facial patch in the eyebrow region. In tuberculoid leprosy, madarosis occurs due to granulomatous infiltration of hair follicles leading to their destruction.
So, if you have a similar paranoia—fear not! Losing strands is totally normal. But Dr. Wexler is quick to add that things like over-tweezing (it causes scarring to the follicles), waxing, severe weight-loss, stress, excessive touching, hormonal changes, and auto immune disease can cause irregular brow hair loss. The majority of women with androgenic alopecia have diffuse thinning on all areas of the scalp. Men on the other hand, rarely have diffuse thinning but instead have more distinct patterns of baldness. Some women may have a combination of two pattern types. Androgenic alopecia in women is due to the action of androgens, male hormones that are typically present in only small amounts. Androgenic alopecia can be caused by a variety of factors tied to the actions of hormones, including, ovarian cysts, the taking of high androgen index birth control pills, pregnancy, and menopause. Just like in men the hormone DHT appears to be at least partially to blame for the miniaturization of hair follicles in women suffering with female pattern baldness. Heredity plays a major factor in the disease.

There is pilosebaceous inflammation[45] with both scarring and non-scarring alopecia depending on the degree of inflammation. Most commonly, there is involvement of the head and neck, though widespread involvement is also seen. Eyebrow loss is a prominent finding and may be the presenting symptom when the eyebrow region is involved in the acute benign form of follicular mucinosis.[46]

The hormonal process of testosterone converting to DHT, which then harms hair follicles, happens in both men and women. Under normal conditions, women have a minute fraction of the level of testosterone that men have, but even a lower level can cause DHT- triggered hair loss in women. And certainly when those levels rise, DHT is even more of a problem. Those levels can rise and still be within what doctors consider "normal" on a blood test, even though they are high enough to cause a problem. The levels may not rise at all and still be a problem if you have the kind of body chemistry that is overly sensitive to even its regular levels of chemicals, including hormones.
Try to include 'hair friendly' foods in your diet, such as Omega 3 rich salmon and walnuts; beat-carotene rich sweet potato, butternut squash, carrots, pumpkin and spinach (yes, dark green veggies are a good source of beta-carotene too); and foods like eggs (rich in biotin, B6, B12, selenium, zinc and copper), lentils (which are loaded with hair-healthy vitamins and minerals), chicken or turkey (which contain lots of protein, B vitamins and minerals), plus vitamin C-rich fruits and vegetables like blueberries, kiwi, strawberries and tomatoes.

Other drugs commonly attributed to causing madarosis are miotics, anticoagulants, anti-cholesterol drugs, antithyroid drugs, propranolol, valproic acid, boric acid, and bromocriptine.[21,99] Anticoagulants in high doses have been found to produce loss of scalp, pubic, axillary, and facial hair with loss of eyebrows after a latent period of a few weeks of treatment with dextran and heparin.[100] Propranolol can cause diffuse alopecia along with loss of eyebrows due to telogen effluvium,[101] usually after three months of therapy.[44] Loss of medial aspect of eyebrows can be seen in fetuses exposed to valproic acid.[102] Diffuse alopecia including that of eyebrows has been described due to chronic ingestion of mouthwashes containing boric acid. There was complete reversal following stopping the practice.[103] Levodopa has been noted to cause severe diffuse alopecia within three months of daily use.[104] Hair loss can occur soon after starting topical minoxidil therapy (due to detachment of club hairs following resting hairs reentering anagen), and after cessation of therapy (due to telogen effluvium).[98]

Toxic alopecia occurs when there is a disruption of hair growth in the anagen phase. This usually occurs following chemotherapy and radiotherapy.[94] Radiotherapy for various types of ocular tumors, eyelid and choroidal tumors have been reported to produce madarosis.[95–97] Hair loss due to radiation is usually reversible, but may be permanent when the dose of radiation is in the range of 50 to 60 Gy.[94]
 Because too much insulin (which is released by the pancreas in large amounts when we consume foods high in sugar or refined flour) signals the ovaries to produce more testosterone, and because an overload of testosterone increases the likelihood that our bodies may make more of the hair-thinning hormone DHT, work to keep blood sugar steady by eating fiber-rich whole foods.  In addition, the Saw Palmetto herb has shown promise for its ability to block the conversion of testosterone into DHT, so it may help androgen-related hair loss. To date, research on saw palmetto for hair loss is limited, but the research that has been done is promising.
Furthermore, Penn dermatopathologists developed an even more advanced method called the HoVert technique for diagnosing hair loss and other disorders from a scalp biopsy. The technique uses a unique horizontal and vertical testing approach that provides a greater amount of information to the referring dermatologist than standard industry longitudinal scalp biopsies.

Changing the shape, thickness and length of the eyebrow is a commonly practiced beauty technique. Some people even remove the eyebrow entirely only to replace it with a pencil-drawn mark (eyebrow pencil). As with any part of the body, repeated irritation can eventually lead to hair loss. Sometimes this is due to manual trauma, chemicals from cosmetics or even infections that arise when breaks in the skin occur. While eyebrow modification will not stop as a commonly practised beauty technique, it should at least be minimized.
Dutasteride is a 5-alpha-reductase inhibitor that binds both types I and II enzymes. Compared with finasteride, its inhibition of type II enzymes is three times more potent; its inhibition of type I enzymes is 100 times more potent (Clark et al., 2004). Dutasteride is not approved for the treatment of FPHL by the FDA, and ongoing studies on the efficacy of the inhibitor are promising but largely focus on male patients (Gupta and Charrette, 2014, Olsen et al., 2006). A study of women after 3 years of therapy showed that dutasteride may be more effective than finasteride in women under 50 years of age as measured by hair thickness (not hair density) at the center and vertex scalp (Boersma et al., 2014). One case report of a 46-year-old female with FPHL showed some response after 6 months of treatment with a dose of 0.5-mg dutasteride daily despite a minimal response to treatment with finasteride and minoxidil (Olszewska and Rudnicka, 2005). Data with regard to the treatment side effects in women is extremely limited. Dutasteride is classified as pregnancy category X because of teratogenicity and should have the same theoretical risk of breast cancer as mentioned in relation to finasteride (Kelly et al., 2016).
